Form used for participating charities to recertify annually that they still meet all Federal and State regulations and that nothing has changed from the prior campaign year.

Once you have submitted the form there is no automatic notification that your submission was received. You will receive a letter of receipt if your submission was received within two weeks of your submission. If you do not receive a letter within two weeks send an e-mail to email@example.com.
To submit the application, click the submit button to check that all fields have been filled in then you will be to save the document to a place on your hard drive, attach it to an email and send it to firstname.lastname@example.org.

This form is used for applicants to apply for the first time or were denied recertification. It is also used for participants looking to change their designation from local campaigns to the statewide campaign or add local campaign areas.
