Application

The deadline for submission for the 2014 SEFA Application has been extended to January 31, 2014. All documentation must be sent together either in an email or postmarked no later than the new deadline.

    Right-click the link above and choose "Save Link As..." to save the form to your computer

Once you have all of the required information complete in the digital Application Form, click on the submit button at the end of the Application Form. Once you have successfully submitted your Application Form you will receive an email confirming your submission was received. If you do not receive an email receipt within 2 hours please contact suzanne@sefanys.org.

 

To submit the application, click the submit button to check that all fields have been filled in then you will be to save the document to a place on your hard drive, attach it to an email and send it to Charities@sefanys.org.

  • This form is used for applicants to apply for the first time or were denied recertification. It is also used for participants looking to change their designation from local campaigns to the statewide campaign or add local campaign areas.

  •  Form used for participating charities to recertify annually that they still meet all Federal and State regulations and that nothing has changed from the prior campaign year.
