Once you have submitted the form there is no automatic notification that your submission was received. You will receive a letter of receipt if your submission was received within two weeks of your submission. If you do not receive a letter within two weeks send an EMAIL to suzanne@sefanys.org.
If, when you click on Submit it does not go into an email for you to send, save the document to a place on your hard drive, attach it to an email and send it to sandy@sefanys.org.

Form used for participating charities to recertify annually that they still meet all Federal and State regulations and that nothing has changed from the prior campaign year.
