Application and Recertification Forms

Application

  •  Form used for applicants to apply for the first time or were denied recertification. Also used for participants looking to change their designation from local campaigns to the statewide campaign or add local campaign areas. Application timeframe as per the SEFA regulations is December 1st through January 15th.

  • Application is put on the website usually in November.

Recertification

  • Form used for participating charities to recertify annually that they still meet all Federal and State regulations and that nothing has changed from the prior campaign year.

  • We will be recertifying one third per year of the total participants over the next three years. Note: In 2009, 2010, and 2011 only 1/3 of participating charities per year will be asked to recertify. A list of those charities required to recertify for 2010 is below.

  • SEFA notified 858 participants of their need to recertify in 2009.

  • In 2010 more then 1300 participants are asked to recertify.
