Application and Recertification Forms

Recertification

This form is the only way a recertification and required documents will be accepted. No hardcopy will be accepted. Recertification takes place in 2015 between March 1 through November 30. The Form only accepts PDF documents uploaded to you Recertification Form. The Recertification Form is used for participating charities to recertify annually. It is a self-certification that the participating charity continues to meet all Federal and State regulations and that all required filings are filed on time.

Form used for participating charities to recertify annually that they still meet all Federal and State regulations and that nothing has changed from the prior campaign year.

The deadline for submission for the 2014 Recertification form is November 30, 2015.

Application

This form is used for applicants to apply for the first time or have failed recertification. It is also used for participants looking to change their designation from local campaigns to the statewide campaign or add local campaign areas.

Once you have all of the required information complete in the digital Application Form, click on the submit button at the end of the Application Form. Once you have successfully submitted your Application Form you will receive an email confirming your submission was received. If you do not receive an email receipt within 2 hours please contact suzanne@sefanys.org.

To submit the application, click the submit button to check that all fields have been filled in then you will be to save the document to a place on your hard drive, attach it to an email and send it to Charities@sefanys.org.
